Small intestine adenocarcinoma accounts for less than 0.5% of all malignant tumors and less than 5% of all gastrointestinal malignancies, making it an extrely rare tumor.

Small intestine adenocarcinoma frequently occurs in the first half of the small intestine, with about 45% occurring in the duodenum, 35% in the jejunum, and 20% in the ileum.

Early-stage cancers are mostly asymptomatic. If there is a lesion on the anal side of the deep duodenum, it cannot be observed as usual through endoscopy. Most patients are found to be in highly deteriorated states due to symptoms like positive fecal occult blood tests, anemia worsening, and abdominal pain and intestinal obstruction caused by intestinal stenosis.

When duodenal lesions progress, in addition to the above symptoms, they block the bile outlet, causing jaundice.

The old man’s condition is considered to be discovered relatively early. Many patients are already in the advanced stage when they go to the hospital for examination after symptoms appear. For these patients, we generally advise against treatnt, especially for older patients, as not treating is often much better than treating.

Moreover, in stage IV and in cases of postoperative recurrence, considering that the tumor has spread throughout the body, chemotherapy is used for systemic treatnt.

Regarding chemotherapy for small intestine adenocarcinoma, past reports have shown that one of the colorectal cancer treatnts, ’fluorouracil oxaliplatin treatnt’ (CAPOX treatnt or FOLFOX treatnt), has good results.

Moreover, the old man’s condition is only at an early stage, so there’s no need for the subsequent combined treatnt plan. Let the old man be completely at ease about that."
